Can Mold Cause Swollen Lymph Nodes?

Swollen lymph nodes turn up on almost every "symptoms of mold exposure" list online. The explanation is usually some version of the same two sentences: your immune system treats mold spores as invaders, and mold toxins overwhelm your lymphatic system.

If you've found a tender lump in your neck and you also have mold in your house, that's an unsettling thing to read.

So can mold actually cause swollen lymph nodes?

The short answer is: yes, but usually not directly. Damp, moldy buildings clearly increase respiratory infections, and respiratory infections are the most common reason the lymph nodes in your neck swell up. Mold can also enlarge lymph nodes through an immune reaction in the lungs, but those nodes sit deep in your chest where you can't feel them.

The mold toxin explanation, the one you'll see repeated most often, doesn't hold up at all.

Here's what the research actually shows, and how to tell which explanation fits your situation.

What makes a lymph node swell in the first place?

A lymph node is a small filter and meeting point for immune cells. Each one sits along the vessels draining fluid from a particular part of your body.

Nodes swell when the immune cells inside them multiply in response to something arriving from the area they drain. The medical term is lymphadenopathy, and the ordinary kind, caused by an immune response rather than cancer, is called reactive lymphadenopathy.

The key thing to understand is that lymph nodes are regional.

The nodes in your neck drain your nose, sinuses, throat and scalp. The nodes deep in your chest drain your lungs. The nodes in your armpit drain your arm, chest wall and breast.

So a swollen lymph node tells you roughly where your immune system is busy. That turns out to be the most useful fact for sorting through the mold claims.

For mold to swell a lymph node, it has to do one of three things: cause an infection in the area that node drains, deliver enough mold particles to that node to provoke an immune response, or trigger a disease that involves lymph nodes directly.

All three can happen. They differ a lot in how common they are, and in which nodes they affect.

Can mold cause swollen lymph nodes through infections?

Yes, and this is the most likely explanation for swollen glands you can actually feel.

The connection works in two steps, and both are well established.

First, damp and moldy buildings increase respiratory infections. A 2010 meta-analysis by researchers at Lawrence Berkeley National Laboratory pooled the published studies on home dampness and mold.

Looking only at studies that controlled for age, sex, smoking and income, people living in damp or moldy homes had 44% higher odds of respiratory infections and 45% higher odds of bronchitis. For children and infants the figure was 48%.

The researchers estimated that somewhere between 8% and 20% of respiratory infections could be attributed to dampness and mold.

Second, respiratory infections swell the lymph nodes in your neck. Every cold, sore throat and sinus infection sends material to those nodes, and they enlarge while your immune system deals with it. A 2016 review in American Family Physician describes swollen lymph nodes as benign and self-limiting in most people, with infection among the leading causes.

Put those together and the logic is straightforward. More mold exposure means more respiratory infections, and more respiratory infections means more swollen glands.

One caveat worth being upfront about: nobody has directly counted swollen lymph nodes in moldy versus dry homes. The dampness research shows an association rather than proving cause. But the effect is consistent across many studies and survives adjustment for the obvious confounders, so it's reasonable to take it seriously.

Can inhaled mold enlarge lymph nodes directly?

Yes, but in lymph nodes you can't feel.

Inhaled mold really does reach your lymph nodes. A 2002 study tracked what happens when spores of Aspergillus fumigatus, one of the most common indoor molds, reach the airways of mice.

Immune cells in the airway swallowed the spores, carried them to the lymph nodes draining the lungs, and instructed other immune cells there on how to respond. The lymph node is where your body organizes its response to anything you breathe in.

In most people that process stays small and invisible. In some, it becomes a disease.

Hypersensitivity pneumonitis is a lung condition caused by repeatedly inhaling something your immune system has become sensitized to, and mold is one of its classic triggers. Farmer's lung, caused by moldy hay, is the textbook example.

A 1996 CT study of 175 people with various lung diseases found enlarged chest lymph nodes in 53% of hypersensitivity pneumonitis patients, compared with 5% of controls. Usually only one or two nodes were enlarged.

But those are mediastinal nodes, buried in your chest. They show up on a scan. You can't feel them, and they won't explain a lump in your neck.

Can mold cause sarcoidosis?

Mold exposure is linked to a higher risk of sarcoidosis, though whether it actually causes it hasn't been established.

This matters here because sarcoidosis is a disease of swollen lymph nodes. It's an inflammatory condition where the immune system forms small clusters of immune cells, most often in the lungs and chest lymph nodes. In the CT study above, 84% of sarcoidosis patients had enlarged chest nodes, the highest rate of any condition examined.

Unlike hypersensitivity pneumonitis, sarcoidosis can also swell nodes in the neck, armpits and groin.

Three separate lines of evidence point toward a mold connection.

The largest is the ACCESS study, published in 2004, which compared 706 newly diagnosed sarcoidosis patients with 706 matched controls across ten US centers. Working in an environment with mold or mildew raised the odds of sarcoidosis by 61%. Working in an area with musty odors raised them by 62%.

Then there's a building cluster. In 2006, investigators from the CDC and the Vermont health department examined a water-damaged office building and found six cases of sarcoidosis. That worked out to 2,206 cases per 100,000 people, against a US range of under 1 to 40 per 100,000. The occupants were relocated.

Finally, a 2011 study found that immune cells from sarcoidosis patients released more inflammatory chemicals when exposed to a component of fungal cell walls. Among those patients, the response tracked with how much fungal material was measured in their bedroom air.

Three different approaches pointing the same way is worth taking seriously. But the limits matter too. ACCESS found an association, not a cause, and its authors said plainly that they hadn't identified a single predominant cause. The cluster is one building. The immune study was small.

And sarcoidosis is rare, so even a 60% increase in odds leaves your absolute risk low. It's a real possibility, not a likely explanation for any one person's swollen glands.

Do mycotoxins swell your lymph nodes?

No. This is the claim you'll see most often, and it has the biology backwards.

The usual version of the claim is that mold toxins build up and overwhelm your lymphatic system, and that the swelling is toxins backing up in the nodes.

But a lymph node doesn't swell because it's full of trapped toxins. It swells because immune cells inside it are multiplying.

And the best-studied mold toxins do the opposite. In a 2002 mouse study, a large oral dose of the mold toxin deoxynivalenol, given alongside bacterial toxin, caused widespread immune cell death in the thymus, bone marrow and gut. Either substance alone did very little.

When mold toxins act on immune tissue at high doses, they deplete it. You'd expect smaller lymph nodes, not bigger ones.

There's also a dose problem. As I worked out in the article on whether mold causes acid reflux, the highest airborne mold toxin levels measured in contaminated buildings come out around 0.0003 micrograms per kilogram of body weight per day. The dose in that mouse study was more than 40 million times higher.

So when mold exposure does lead to swollen lymph nodes, what you're seeing is an immune response, to an infection or to mold particles. The swelling itself isn't toxins accumulating in the node.

That's a point about why nodes get bigger, not an argument against reducing your mycotoxin body burden. Lowering body burden and shrinking a lymph node are two different goals, and binders have their own evidence base, which I'll cover separately.

Can a mold allergy cause swollen lymph nodes?

Rarely on its own, but a mold allergy often sets up the infections that do.

Allergic inflammation in your nose is driven by immune cells in the lining, and it usually doesn't produce nodes large enough to feel.

What a mold allergy reliably does is block your nose and sinuses. Blocked sinuses stop draining properly, bacteria grow in the trapped mucus, and the resulting sinus infection swells the lymph nodes in your neck.

So if your glands swell during allergy flares, an infection riding on top of the congestion is the more likely culprit.

Can a fungal infection cause swollen lymph nodes?

Yes, but household mold almost never causes one if your immune system is working normally.

The fungal infections known for swollen lymph nodes are histoplasmosis, coccidioidomycosis (valley fever) and cryptococcosis. These come from soil, dust, and bird or bat droppings in particular regions of the country, not from mold growing on your drywall. They mainly affect the lymph nodes in your chest.

Indoor molds like Aspergillus can invade tissue in people whose immune systems are severely suppressed, after a transplant or during chemotherapy. For everyone else, household mold is an allergen and an irritant, not an infection.

Can CIRS cause swollen lymph nodes?

People with CIRS do report swollen lymph nodes, but lymph nodes have never been measured in a CIRS study, so there's no data on how common it is or which nodes are involved.

The biology makes it plausible. CIRS involves ongoing immune activation, and that kind of inflammatory signaling is exactly what makes lymph nodes enlarge. It increases blood flow into the node and pulls in more immune cells.

You'd expect that to produce small, persistently reactive nodes in the areas draining the airways. But that's reasoning from mechanism, not an observation, and the study that would settle it is a straightforward neck ultrasound comparison that nobody has run.

Can black mold cause swollen lymph nodes?

Black mold can cause swollen lymph nodes the same way any other indoor mold can, and there's nothing in the research showing it's special.

The infection data covers damp and moldy homes generally. The direct immune research involves Aspergillus and the mixed molds behind hypersensitivity pneumonitis, all of which are far more common indoors than Stachybotrys.

Where black mold does matter is as a signal. It needs constantly wet material to grow, so finding it tells you the building has a serious, long-standing water problem, usually with plenty of other microbial growth alongside it.

Can mold cause swollen lymph nodes in your armpit?

Mold can plausibly explain swollen nodes in your neck. It doesn't explain swollen nodes in your armpit.

This comes back to drainage. Mold enters through your nose, throat and lungs, and those areas drain to the lymph nodes in your neck and chest.

Your armpit nodes drain your arm, chest wall and breast. Your groin nodes drain your legs and pelvis. Nothing you breathe in sends material to either.

The one exception is a body-wide condition like sarcoidosis, which can enlarge nodes in several areas at once.

Do swollen lymph nodes go down after leaving a moldy building?

Probably, if mold was the cause, though nobody has tracked it directly.

No study has measured lymph nodes before and after remediation or a move. But the mechanisms make a clear prediction.

If your swollen nodes come from repeated respiratory infections, they should become less frequent as the infections do. Hypersensitivity pneumonitis is known to improve once you're away from the trigger, which is why the investigators of the Vermont sarcoidosis cluster recommended relocating the building's occupants.

A node that stays enlarged for weeks after the exposure and the infections have ended isn't behaving like a reactive node, and mold probably isn't the explanation.

How can you tell whether mold is behind your swollen lymph nodes?

The pattern usually points to the answer.

Small, soft, tender nodes in your neck that come with a cold or sinus infection and shrink over the following weeks are reactive nodes from an infection. If those infections keep coming in a damp building, mold is a plausible upstream cause.

Cough, breathlessness and fatigue that ease when you spend days away from a building and return when you go back point toward hypersensitivity pneumonitis, which needs a chest scan to identify.

Swollen nodes in two or more areas, a node just above your collarbone, or swollen nodes alongside fever, night sweats or unexplained weight loss are a different category entirely. The American Family Physician review lists exactly those features, along with being over 40, as warning signs for cancer.

Mold doesn't explain that last pattern, and assuming it does is how lymphoma gets missed. That combination needs proper medical assessment regardless of what's growing in your building.

The single most useful thing to pay attention to is timing. If your symptoms consistently ease when you're away from a building and return when you go back, the building is worth investigating, whichever mechanism turns out to be responsible.
